Privacy Policy
Base Pelvic Health and Physiotherapy (the "Clinic") is committed to protecting the privacy, confidentiality, and security of your personal health information (PHI). This policy outlines our practices in accordance with the Personal Health Information Protection Act, 2004 (PHIPA) and the Personal Information Protection and Electronic Documents Act (PIPEDA).

Primary Care: To provide safe pelvic physiotherapy assessment and treatment.
Circle of Care: To communicate with other healthcare providers involved in your care (e.g., your family doctor or referring specialist) unless you instruct us otherwise.
Administration: To invoice for services, process insurance claims, and collect unpaid accounts.
Quality & Regulation: To comply with the regulatory requirements of the College of Physiotherapists of Ontarioand the Regulated Health Professionals Act (RHPA).
Legal Requirements: To respond to legal proceedings or as otherwise required by law.
3. Your Rights
You have the following rights regarding your health record:
Access & Correction: You have the right to see and receive a copy of your PHI and to request a correction if you believe the record is incomplete or inaccurate.
Withdrawal of Consent: You may withdraw or limit your consent for the use or disclosure of your PHI at any time (subject to legal or contractual restrictions and reasonable notice).
Inquiries: You may request a full copy of our Privacy Policy at any time.
4. Safeguards and Retention
We employ physical, technical, and administrative safeguards (such as encrypted software and secure filing) to protect your information from theft, loss, and unauthorized access. We retain and destroy your PHI in strict accordance with the regulations set by the College of Physiotherapists of Ontario.
